Document List for Patient Referrals

The following is a list of documents that we need to have listed by type of appointment:

Ultrasound:

renatal records, all previous ultrasound reports (if available), a copy of patient’s insurance card and a completed copy of the Sac MFM referral form.

Ultrasound Consult:

Chart notes, all previous ultrasound reports (if available), a copy of the patient’s insurance card, and a completed copy of SacMFM referral form.

Maternal-Fetal Medicine Consult:

Prenatal records, records pertinent to patient’s diagnosis, prenatal lab results, ’s insurance card and a completed copy of the Sac MFM referral form.

Genetic Counseling:

Prenatal records, all previous ultrasound reports (if available), prenatal labs, a ’s insurance card and a completed copy of the Sac MFM referral form.

Follow-up of abnormal California Prenatal Screening Program Test Result:

Prenatal records, all previous ultrasound reports (if available), prenatal labs, a copy of patient’s insurance card and a completed copy of the Sac MFM referral form
